Some orthodontic problems start with the shape of the upper jaw, not the teeth. When the upper jaw is too narrow, Dr. Tewes may recommend a palatal expander to create space and help the bite line up.

What Is a Palatal Expander?

A palatal expander is a small, custom-made appliance fitted to the roof of the mouth. It has two halves joined in the center by a screw. Turning the screw with a special key on a set schedule gently widens the upper jaw over time.

The upper jaw grows in two halves that don’t fully join until closer to puberty. That’s why expansion works best while a child is still growing, although some teens and adults can benefit too.

Phase 1 Treatment & Expanders

Dr. Tewes may recommend an expander when there is:

  • Not enough room for permanent teeth coming in
  • A crossbite with a narrow upper arch
  • Crowding that could lead to extractions later

Expanders are often part of Phase 1, or early interceptive treatment. Phase 1 helps the jaw grow so it can make room for your child’s permanent teeth. Widening the upper jaw early can gain space, lower the chance of impacted teeth and, in some cases, simplify treatment later on.

Airway, Sleep & Your Bite

Your bite and the way your jaws line up affect more than your smile. Bite problems can play a part in snoring, teeth grinding, and jaw pain.

What to Expect

On average, patients wear a palatal expander for four to seven months, depending on how much correction is needed. It stays in place the whole time so the bone can form at its new width.

An expander doesn’t always remove the need for braces later, but it can make future treatment simpler. The American Association of Orthodontists recommends a first orthodontic check-up by age 7.
